INTRODUCTION : A 75-year-old male patient visited our clinic seeking a comprehensive solution for long-standing dental problems that had significantly affected his quality of life. He was presented with a severely collapsed bite, extensive enamel loss, and multiple worn and attrited teeth, making it difficult for him to eat comfortably or smile with confidence. Following a detailed assessment, a phased treatment plan involving root canal therapy, temporary bite-raising crowns, final zirconia crowns, and a removable partial denture was planned. PATIENT BACKGROUND : The patient, a 75-year-old male, had previously undergone dental treatments at multiple clinics but remained dissatisfied due to ongoing discomfort and functional limitations. INITIAL ASSESSMENT & DIAGNOSIS : Clinical examination revealed generalized tooth wear, fractured enamel, and multiple attrited teeth across both arches. The patient had a significantly reduced vertical dimension due to long-term wear, leading to a collapsed bite. Several teeth showed compromised vitality, requiring endodontic intervention to preserve them for long-term restoration. The lower central incisors were deemed non-restorable and indicated for extraction.
Radiographic evaluation supported the need for extensive root canal therapy in multiple teeth and full-coverage restorations with all ceramic metal free zirconia crowns . Given the patient’s preference to avoid implants, a removable partial denture was planned to replace missing posterior teeth in the lower arch. A comprehensive full mouth rehabilitation was recommended to restore proper occlusion, aesthetics, and oral function.

TREATMENT PLAN :

A customised full mouth rehabilitation treatment plan in Delhi was formulated with the following objectives: restoring the vertical dimension, preserving natural teeth, improving aesthetics, and achieving long-term functional stability. The plan included:

  • Single-sitting root canal treatments to save teeth with compromised enamel and pulp involvement

TREATMENT PROCESS :

  • 19 May 2025: Root canal treatments were carried out on the lower right lateral incisor, canine, first premolar, and second premolar, as well as the lower left lateral incisor, canine, first premolar, and second premolar. Crown preparation was completed from the upper left second molar to the upper right second molar and from the lower right second premolar to the lower left second premolar. The lower central incisors were extracted.
  • 20 May 2025: Root canal therapy was completed for the upper right first and second molars, both upper central incisors, and the upper left first and second molars.
  • 22 May 2025: Temporary crowns were cemented on all prepared teeth to raise the bite and evaluate patient comfort.
  • 24 May 2025: A specialist follow-up confirmed proper adaptation and comfort with the temporary restorations.
  • 27 May 2025: Adjustments were made to the length of the upper central and lateral incisors based on the patient’s aesthetic preferences.
  • 30 May 2025: Final impressions were taken for the fabrication of permanent zirconia crowns.
  • 6 June 2025: Permanent zirconia crowns were cemented, and jaw relations were recorded for the removable partial denture.
  • 10–14 June 2025: The removable partial denture and night guard were delivered, followed by multiple follow-ups and adjustments to ensure optimal comfort and function.

RESULTS & OUTCOME :

The full mouth rehabilitation resulted in a dramatic functional and aesthetic transformation. The patient’s vertical dimension was successfully restored, improving facial harmony and bite stability.
